How do I become a patient?

To get started on the healing journey, we ask you to fill out the New Patient Inquiry Form and tell us about your health concerns. Once you fill out the form, a patient care coordinator will be in touch with you within a few days to make sure that the care we provide fits your health needs.

What are the different services you offer?

At The UltraWellness Center, we offer the following services:

1. Functional Medicine Consultations: 

Your health journey will begin with an initial consultation over the phone with a Physician Assistant to learn more about your health history and background. Next, you will meet with your physician by traveling to our clinic in Lenox, Massachusetts. Each new patient experience is designed to cover everything from an extensive health intake, through innovative lab testing and nutrition consultations.

2. Functional Nutrition Consultations:

Functional Nutrition is a wonderful entry point to Functional Medicine. Working with our nutritionists is a great way to get started on your health journey. 

Our nutritionist can run Functional Nutrition lab testing on you so they can personalize your diet. In this blog post you can learn more about how Functional Nutrition could potentially help you heal.

3. IV Therapy:

We offer intravenous therapies that will support your general wellbeing, treat nutrient deficiencies, as well as support your healing journey at The UltraWellness Center. While we strongly believe in a food-as-medicine approach to healing, we also understand the value a “flood of nourishment” provides to patients healing from chronic illness and those seeking a return to optimal health. IV infusions and nutrient injections filled with vitamins, minerals, and other nutrients can often be the turning point to spark a reversal of symptoms for those with chronic illness, or those looking to optimize their health and performance.

4. Ozone/Prolozone Therapy:

Oxygen-ozone therapy has a long history of safe use to support and improve health. It can support the body to activate its own healing mechanisms, including inhibiting inflammatory pathways, activating anti-inflammatory pathways, and increasing your body’s own antioxidant and detoxification systems to improve overall health.

Prolozone therapy is a safe and effective way to address chronic musculoskeletal complaints and support the regeneration and repair of joints and tissues.

Do you accept insurance?

The Ultrawellness Center does not participate with any insurance carrier. We cannot submit medical claims on your behalf, nor assist you with claim resolution. You are responsible for submitting any claims to your insurance company.

All services are strictly on a self-pay basis; however, we will provide you with an insurance billing invoice that you can submit to your insurance carrier for possible reimbursement.

Insurance billing invoices for laboratory tests may not be given until the laboratory tests have been resulted and received by The Ultrawellness Center.

Please keep in mind that there may be procedures and laboratory tests that are non-covered due to your individual insurance policy or plan. Should you have any questions regarding your medical coverage, please contact your insurance carrier. Their telephone number should be on the back of your card.

Your insurance company may request additional information and may not accept our diagnosis codes or may deny your claim. It is important to note that the Functional Medicine model of care is not a conventional model of care and often not covered by insurance. The conventional model is based on using and labeling patients with reimbursable diagnostic codes. These conventional reimbursable codes are not always consistent with the functional medicine assessment and may not be covered. Unfortunately, we cannot change any medical codes nor communicate with your insurance company.

Please notify your insurance company that any payments should be sent directly to you.

Please be advised insurance billing invoices will not be provided for any remote or telemedicine service.

Our physicians and nutritionists are not Medicare providers: therefore, your consultation and some laboratory charges from our office are not reimbursable by Medicare.

Is Dr. Mark Hyman accepting new patients?

Dr. Mark Hyman is not accepting new patients at this time. To work with one of The UltraWellness Center physicians trained by Dr. Hyman, click here.

 

Do I have to travel to Lenox, Massachusetts for a Functional Medicine appointment?

To work with a Functional Medicine physician you will be required to come to The UltraWellness Center in Lenox, Massachusetts for your initial consultation and maintain a relationship with a primary care physician. Follow-Up appointments can be done remotely; however, patients are required to be seen in-office on a yearly basis.

 

 

Can the physicians at The UltraWellness Center be my primary care physician?

No, we require that you have your own primary care physician. While our physicians are trained as primary care physicians, they do not provide acute care services. We will work with you closely as consultants and coaches in preventive, nutritional and functional medicine to help you address the roots of chronic health problems. They can confer with your primary care doctor if required.

What do I need to do before my appointment?

Please fill out the medical questionnaire/consent forms that pertain to your appointment type. You can find these forms on your patient portal.

Patient Guidelines for Medical Record Submissions

As you begin your journey to health at The UltraWellness Center, we begin a relationship together that will take course over the months and/or years to come. As inherent in the Functional Medicine Model, we will want to understand the complexity of who you are, your story, the triggers and precipitating factors that have impacted your health, and where this has brought you to date. This discovery process is part of us getting to know you, your history and your needs, and helps us formulate a treatment plan that is specifically catered to you. There are many facets to this initial discovery process. You will be asked to complete an online questionnaire upon scheduling your appointment. You may be asked to have a Pre-Visit appointment with a Physician Assistant so we can focus more specifically on the information needed for your first appointment with the doctor. To better help you understand what medical records are useful, we have outlined the following guidelines:

  • Medical summary within the last 12 months from your Primary Care Provider
  • Conventional lab results within the last 3-6 months
  • Functional Medicine test results within the last 12 months

Items we ask that you DO NOT SEND:

  • Notes from routine medical office visits
  • Routine exam notes (this includes immunizations, routine physical exams, etc.)
  • Lab results that are over a year old
  • Diagnostic imaging films or disks

Please do not submit screenshots of your medical records. We require copies of your original medical documents.

* While we recognize that everyone is unique in the journey that has brought you to us, so can this apply to the information that may be pertinent. Please remember, more is not always better. If you request multiple past medical records to be reviewed by our physician, an additional fee will be applied.

Do you work with cancer patients?

Our doctors work with patients to help create an environment in their body where cancer is less likely to grow. For those who are in treatment we work with the oncologist team and focus on nutrition and lifestyle modification to strengthen the immune system, enhance detoxification and support the patient through their treatment. Post treatment, we work to decrease the risk of recurrence.

Do you work with patients that have autoimmune disease?

Yes, we work with patients who have Autoimmunity and have seen great improvements using our Functional Medicine approach.

Do you work with children?

Dr. Boham and Dr. Papanicolaou and all of our nutritionists work with children under the age of 13. Dr. LePine works with children ages 13 and older. Dr. Geyer works with patients 18 and older.

How do I get to The UltraWellness Center?

Lenox, Massachusetts is located about 2 hours west of Boston (MA) and 3 hours north of New York (NY). For air travel: Albany International Airport (ALB) is approximately 50 minutes from our office and Bradley International Airport (BDL) is approximately 75 minutes from our office.

What is the best way to get around Lenox?

We highly recommend renting a car to get around.

What are your rates?

The following Pricing Page will give you a breakdown of the services we provide.

What is your cancellation policy?

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment schedule.

FUNCTIONAL MEDICINE NEW PATIENT APPOINTMENTS:

Due to the overwhelming requests for Functional Medicine consultations, along with all the work that goes into preparing for an initial consultation (reviewing intake forms, background intake with a PA, scheduling travel, etc.), new patient appointments must be secured by a $500 deposit and must be canceled or rescheduled no later than 4 weeks prior to your scheduled physician appointment or it will result in the forfeit of the deposit.

For all Functional Nutrition new patient appointments canceled within 1 week of the appointment date, there will be a $145 cancellation fee due immediately upon cancelation.

For all Follow Up appointments there is a 72-business hour cancellation policy. If you cancel with fewer than 72 business hours, you will be charged 100% of the appointment fee.
