PATIENT NAVIGATOR

Job Summary:

Establishes and maintains effective relationships with patients by scheduling appointments and responding promptly to their needs. Works with patient and clinical team to help facilitate and resolve patient experience issues through effective communication and problem solving.

Reports To: Patient Navigator Supervisor

 

Essential Functions/Position Responsibilities:

Acts as first point of contact (liaison) for all patient concerns.

* Listen to the issue

* Escalate to proper team member

* Track concerns and resolution times

* Identify patterns and escalate to supervisor

  • Answers and directs incoming calls promptly
  • Attends team meetings prepared with questions and answers needed
  • Schedule 1st follow up appointments utilizing High Rise to ensure that all lab tests have been completed and resulted prior to appointment
  • Schedules and reschedules new and existing patient appointments as needed
  • Connects with in office new patient on day of appointment to introduce themselves as their main point of contact
  • Connects with each new patient 2 weeks after their initial appointment to ensure that they are feeling supported and understanding their new plan
  • Provides information regarding The UltraWellness Center, Functional Medicine approach, pricing, and policies
  • Ensures all communication received is responded to in a timely manner
  • Assists with any other office duties and projects as needed

 

Back-Up Responsibilities:

  • Acts as back up to all other Admin team members for planned, unplanned absences, and as volume determines

 

UWC Compliance:

  • Complies with all OSHA regulations
  • Complies with all HIPAA Privacy and Security Laws
  • Maintains a safe and clean working environment

 

Required Experience and Skills:

PRACTICE MANAGER

Job Summary:

Manages and directs day-to-day operations of Functional Medical practice. Involved in a broad range of critical activities, ranging from strategic to tactical, and plays a key role in helping the organization focus on, and achieve, its critical business goals.

Reports To: Dr. Hyman and CEO

Essential Functions/Position Responsibilities:

  • Direct, supervise, and evaluate work activities of medical, nursing, laboratory, nutrition, administrative, and other staff.
  • Provides leadership, coordination, and oversight in the development of strategic initiatives and special projects for the organization.
  • Plans, implement, administrate programs and services including human resources, training, and coordination of clinical and administrative staff. Develops employee and UWC policies and procedures.
  • Ensures legal compliance and adheres to accreditation requirements according to state and national laws.
  • Establishes goals and objectives and evaluative or operational criteria for The UltraWellness Center.
  • Provides integrated analysis and strategic consultation on major issues impacting the organization and reports findings to Leadership team.
  • Manages fiscal operations for The UltraWellness Center including planning budgets, authorizing expenditures, establishing rates for services, and coordinates financial reporting.
  • Approve payroll paying close attention to cost and budget.
  • Directs and conducts recruitment, hiring, and training staff. Establishes work schedules and assignments for staff according to workload, space, and equipment availability.
  • Maintains awareness of advances in conventional and functional medicine, computerized diagnostics and treatment equipment, medical records management, government regulations, health insurance changes, and financial options.
  • Offers innovative solutions to increase revenue and bring in new clientele.
  • Inspects facility and recommends building or equipment modifications to ensure emergency readiness and compliance to access, safety and sanitary regulations.
  • Anticipates and resolves potential problems with patients.
  • Serves as HIPAA and OSHA Compliance Officer.

Required Experience and Skills:

  • Demonstrate understanding of health care environment.
  • Ability to define priorities and assign resources in pursuit of short-and long-term goals.
  • Recruiting, hiring, and onboarding experience.
  • Knowledge of quality assurance best practices.
  • Budgeting and negotiations skills.
  • Project management experience required.
  • High level of computer proficiency, specifically with Outlook, Microsoft Word, Excel, and electronic medical records.

 

Training and Educational Requirements:

  • Bachelor’s degree in healthcare management or related field required, Master’s degree preferred.
  • Minimum of 5 years management experience in the healthcare field.
  • Knowledge of healthcare regulations.

FUNCTIONAL MEDICINE PHYSICIAN ASSISTANT

Job Summary:

The physician assistant is responsible for seeing patients for a variety of health conditions, utilizing the principles of functional medicine for diagnosis, and determining treatment protocols, while working closely with providers, nurses, nutritionists, and administrative team.

Reports To: Medical Director & Supervising Physician

 

Essential Functions/Position Responsibilities:

  • Effectively provide functional medicine clinical services to include assessment, diagnosis, treatment, and referral for initial patient appointments, follow up appointments, and patient needs between appointments.
  • Issue appropriate prescriptions, laboratory tests, and supplements as needed.
  • Accurately and timely complete documentation of above clinical services in the electronic medical record.
  • Collaborates in multidisciplinary coordination of care, including counseling and education on health behaviors.
  • Provides patient assessments and IV recommendations for Community IV Therapy Program.
  • Maintain clinical practice according to relevant national and state practice guidelines.
  • Consults with Medical Director and Supervising Physician concerning continuous quality improvement measures.
  • Actively participate in performance improvement activities, practice development and other duties which support the mission of The UltraWellness Center on an as needed basis.
  • Participates in Medical Staff, Clinical Rounds, and Clinical Strategic Planning meetings.
  • Adhere to HIPAA Privacy and Security regulations mandated by the Federal Health and Human Service Agency.
  • Contribute to ultrawellnesscenter.com by writing case study blogs and expert staff blogs.
  • Follow all policies, procedures, and protocols set by practice management.
  • Fulfill all practice contract obligations.
  • Represent The UltraWellness Center in a professional manner within the local community and within any community you visit.

 

Required Experience and Skills:

  • Strong clinical knowledge base required.
  • Be in alignment with functional medicine and our work at The UltraWellness Center.

  • Physician Assistant license to practice in the Commonwealth of Massachusetts.
  • Minimum of 5 years’ experience as a Physician Assistant in medical practice.
  • Maintain current BLS certification.
  • Commitment to ongoing functional medicine knowledge/training.
  • Certification from The Institute for Functional Medicine, preferred.
