FUNCTIONAL MEDICINE IV RN

Job Summary: 

Assists in the ongoing development of IV protocols and therapies.  Administers IV therapy to patients as prescribed by a UWC provider.

 

Reports To:  Practice Manager

 

Essential Functions / Position Responsibilities

  • Establishes a positive, trusting relationship with patients (and their caregivers where appropriate), communicating clearly and kindly with focus on comfort of patient and their well-being
  • Administers IV therapies per UWC protocols as prescribed by providers.  Administration includes:  proper mixing of sterile ingredients according to compounding safety standards, IV insertion, monitoring patients during infusion, patient education and documentation in EMR, and proper disposal of medical wastes
  • Collaborate with the Medical Director in the ongoing development of IV therapy protocols
  • Maintains standards of practice for IV therapy program and lab
  • Assesses patient response to medications/nutritionals:  adverse reactions and side-effects, as well as issues with IV placement
  • Provides education regarding medication administration, medication-supplement interactions; changes in medications, communications regarding medication modifications, etc.
  • Assists MD with medical treatments when necessary
  • Manages IV lab, supplies, equipment, medications and nutritional supplements, ordering supplies as indicated in collaboration with LPN/ lab personnel, ensures expiration dates are current, supplies are stored in accordance with guidelines
  • Collaborates with UWC clinical team (physician, physician assistant, nutritionist, nurse, and other staff as needed) and coordinates care between patients and providers, utilizing clear communications
  • Prepares for IV Therapy patient appointments for both new and established patients: reviews patient’s medical history and treatment plans
  • Responds to patient inquiries, concerns, requests, and complex needs, in a timely manner
  • Informs and recruits IV patients for IFM research project
  • Maintain accurate records for research project and ensure surveys are sent out on time
  • Communicates new updates in treatment plan at provider request
  • Ensures continuity of care through on-going navigational support and provision of patient education, utilizing identified tools and educational handouts at strategic points to enhance patient knowledge and understanding of treatment process
  • Documents clinical information and communications accurately and in timely manner in electronic medical record reflecting services and care coordination provided, and plans for future steps needed
  • Responds to e-mails, voice mails, and other messages in a timely manner
  • Assists with projects as directed by the Leadership Team
  • Attends weekly Clinical Rounds and other UWC meetings as requested

 

UWC compliance

  • Maintains confidentiality of necessary information, by adhering to all HIPPA Laws
  • Maintains safety standards by adhering to OSHA rules and regulations
  • Adheres to all UWC clinical practices and protocols

 

Required Experience and Skills

  • IV therapy experience required
  • Exceptional customer service and communication skills
  • Capable of working independently and cohesively with a team
  • Strong clinical skills: clinical judgement/decision making, assessment skills, ability to intervene within scope of practice and facilitate smooth patient care coordination
  • Ability to maintain a high level of professionalism when dealing with difficult/demanding personalities
  • Maintain the highest level of confidentiality and adhere to all HIPPA Laws
  • High level of computer proficiency, specifically with Electronic Medical Records, Outlook, Microsoft Word, and Excel
  • Experience with electronic medical records
  • Organized and detail oriented
  • Able to multitask
  • Able to maintain accurate, detailed reports and records
  • Experience or knowledge of Functional Medicine, Integrative Medicine, or similar alternative/holistic therapies
  • Minimum of 1 year working as a registered nurse

 

Training and Educational Requirements

  • Education:  BS or BSN
  • Current Registered Nurse license to practice in State of Massachusetts
  • Current CPR license
  • Training through The Institute for Functional Medicine (IFM) preferred, which may include:  Functional Medicine Coaching Program; AFMCP; Advanced Modules

LICENSED PRACTICAL NURSE

Job Summary: 

Coordinates care between patients and UWC clinical team (physician, physician assistant, nutritionist and other staff as needed) utilizing clear communications while addressing patients’ clinical needs.

 

Reports To:  Practice Manager

 

Essential Functions / Position Responsibilities

New Patient / In House Patient Care

  • Meet with patient to review lab testing ordered by provider
  • Researches and identifies needed resources in patient’s community in accordance with treatment plan to ensure coordination of care, and assists with communications with these resources as appropriate

 

Remote LPN Care

  • Manages medication refill needs, assisting patients with strategies/resources for obtaining necessary medications as warranted (including international patients)
  • Provides continuity of care including coordination of special requests and patient needs
  • Medical record review
  • Monitor RX line and process refill requests
  • Prior-authorizations

 

Administrative Duties

  • Documents clinical information and communications accurately and in timely manner in electronic medical record reflecting services and care coordination provided, and plans for future steps needed
  • Responds to e-mails, voice mails, and other messages in a timely manner
  • Assists with projects as directed by Clinical Care Coordinator
  • Attends weekly Clinical Rounds and other UWC meetings as requested
  • Serves as resource for medial team regarding lab testing

 

Back-Up Responsibilities

  • Blood draws, processing, and kit preparation
  • Assess patient’s vital signs and other preliminary information as needed
  • Review, with patient, specialty kit instructions, medications/prescriptions, completion of consent forms, identification of lab/ other testing resources in their home community, and other issues requiring action steps to ensure smooth coordination of care (including provision for international needs)
  • Assist RN’s as needed with patient communication and care

 

Compliance

  • Maintains confidentiality of necessary information, by adhering to all HIPPA Laws
  • Maintains safety standards by adhering to OSHA rules and regulations
  • Adheres to all UWC clinical practices and protocols
