Becoming a patient

How do I become a patient?

To get started on the healing journey, we ask you to fill out the New Patient Inquiry Form and tell us about your health concerns. Once you fill out the form, a patient care coordinator will be in touch with you within a few days to make sure that the care we provide fits your health needs.

What are the different services you offer?

At the UltraWellness Center, we currently offer four different services- two of which can be done remotely.

1. Functional Medicine Consultations:

This is not your typical doctor’s visit. This is a partnership between us as we work together to create the path to wellness. Your health journey begins with an Initial Consultation over the phone with a nurse to learn more about your health history and background. You will then travel to our clinic in Lenox, Massachusetts for a day of health, lifestyle and nutrition assessments. Each new patient appointment is designed to cover everything from an extensive health intake, through innovative lab testing and nutrition consultations.

To work with a Functional Medicine physician you will be required to come to The UltraWellness Center in Lenox, Massachusetts for your initial consultation and maintain a relationship with a primary care physician.

Please find more details by filling out our New Patient Inquiry Form.

2. Functional Nutrition Consultations:

For those who are unable to travel to our clinic, we do offer Functional Nutrition Consultations which can help many people. Functional Nutrition is a wonderful entry point to Functional Medicine. Working with our nutritionists is a great way to get started on your health journey.

Our Nutritionist can run Functional Nutrition lab testing on you so they can personalize your diet. In this blog post you can learn more about how Functional Nutrition could potentially help you heal.

Please find more details by filling out our New Patient Inquiry Form.

3. IV Therapy

We offer intravenous therapies that will support your general wellbeing, treat nutrient deficiencies, as well as support your healing journey at The UltraWellness Center. While we strongly believe in a food-as-medicine approach to healing, we also understand the value a “flood of nourishment” provides to patients healing from chronic illness and those seeking a return to optimal health. IV infusions and nutrient injections filled with vitamins, minerals, and other nutrients can often be the turning point to spark a reversal of symptoms for those with chronic illness, or those looking to optimize their health and performance.

You can learn more about IV Nutritional Therapy here.

Do you accept insurance?

We do not accept insurance or Medicare. Some insurance carriers may cover medical services and laboratory tests performed by our clinicians. We can provide a detailed receipt for services performed for you to submit to your insurance carrier directly.

Patient Guidelines for Medical Record Submissions

As you begin your journey to health at the UltraWellness Center, we begin a relationship together that will take course over the months and/or years to come. As inherent in the Functional Medicine Model, we will want to understand the complexity of who you are, your story, the triggers and precipitating factors that have impacted your health, and where this has brought you to date. This discovery process is part of us getting to know you, your history and your needs, and helps us formulate a treatment plan that is specifically catered to you.  There are many facets to this initial discovery process. You will be asked to complete an online inventory upon scheduling your appointment. You will also be asked to have a Pre-Visit appointment with a nurse or physician assistant so we can focus on more specifically on the information needed for your first appointment with the doctor. In addition, we ask that you also provide us with pertinent medical records that may be important for us to better understand your medical situation. In order to better help you understand what medical records are useful, we have outlined the following guidelines.

  • Most recent medical summary from your latest medical provider(s). This may include specialists you have seen for pertinent evaluations. (Please do not send a series of medical office notes.  An overview/summary describing most recent findings would be very valuable.)
  • Recent lab results from the past 6 months
  • Other medical test summaries from the past year
  • Medical records summary from your provider that reflects a significant medical/surgical event that may have triggered a change in your status
  • Summary of current (past 6 months) treatment regimens/therapies
  • Genetic testing results
  • Recent Functional Medicine test results (within the past one year)
  • Mental health/psychiatric summary

Items that we ask that you DO NOT SEND:

  • Notes from routine medical office visits
  • Routine exam notes (this includes immunizations, routine physical exams, etc.)
  • Lab results that are over a year old
  • Diagnostic imaging films or disks

**While we recognize that everyone is unique in the journey that has brought you to us, so can this apply to the information that may be pertinent.  Please remember, more is not always better.  However, if there is something not included on this list that you feel is relevant to your care with us and should be included, you can speak with the practitioner at the time of your Pre-Visit to find out if it’s something that would be beneficial to send.

Dr. Hyman explains why it’s important to get your supplements from a reliable source:

Thank you for choosing our store for your supplement needs. At our online store you will find a wide array of products my team and I hand-picked to address specific conditions and cover the nutrient bases you might not be getting even if you’re eating fresh, whole, real foods.

Buying quality supplements from the right sources is so important because unlike pharmaceutical drugs, supplements are not regulated by the U.S. Food & Drug Administration (FDA). The manufactures I carry in my formulary have strict criteria. All the manufactures adhere to Good Manufacturing Practices standards.

When choosing supplements it is important to be careful of third party vendors, online, and especially big box retailers. We order direct from most manufactures and guarantee our quality control regarding temperature and storage.

Buying quality supplements is important, but buying them from the right source also becomes crucial to get the freshest, highest-potency nutrients.

When you shop with us, you can rest assured every product is guaranteed to be safe, effective, and fresh. Our customer support team is here to help you with anything you need, so please don’t hesitate to reach out if you need additional help.

Wishing you health and happiness,

Mark Hyman, MD

Rates and policies

What are your rates?

The following Pricing Page will give you a breakdown of the services we provide.

What is your cancellation policy?

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment schedule.

For all Functional Medicine Initial Consultations canceled within 3 weeks of the appointment date, there will be a cancellation fee of $500 due immediately upon cancelation.
– The fee will be applied to any New Patient Appointment that is rescheduled within one (1) year from original appointment date.

For all Functional Nutrition New Patient appointments canceled within 1 week of the appointment date, there will be a 25% cancellation fee due immediately upon cancelation.

For all Follow Up Appointments there is a 72-hour cancellation policy. If you cancel with fewer than 72 BUSINESS hours, you will be charged 100% of the appointment fee.

We appreciate your understanding.
